In this Contract, the word “You” and “Your” means each and all of the people named as owner and who sign this Contract.  

CONTRACTOR’S NAME    1st American Builders of Chicago Inc. The words “I”, “My” and “me” mean

GOODS AND SERVICES: You agree to purchase from me, and I agree to sell to you, the following goods and/or services.

THE LAW REQUIRES THAT THE CONTRACTOR SHALL SUBMIT A SWORN STATEMENT OF PERSONS FURNISHING MATERIAL AND LABOR BEFORE ANY PAYMENTS ARE REQUIRED TO BE MADE TO THE CONTRACTOR.

This contract is subject to the terms on the reverse side, and they are part of this Contract.

I have a lien for services, labor and material on the property at the job address and on all buildings located thereon in accordance with Illinois Revised Statutes, Chapter 82, Paragraphs 1 through 39.

NOTICE TO OWNER

Do not sign this contract before you read it or if it contains any blank spaces.  You are entitled to an exact copy of the agreement you sign.

NOTICE OF CANCELLATION

YOU, THE CONSUMER, MAY CANCEL THIS TRANSACTION AT ANY TIME PRIOR TO MIDNIGHT OF THE THIRD BUSINESS DAY AFTER THE DATE OF THIS TRANSACTION.  SEE THE ATTACHED NOTICE OF RIGHT OF CANCELLATION FORM FOR AN EXPLANATION OF THIS RIGHT.

By signing below, you acknowledge receipt of a copy of this Contract.  Any co-signer also acknowledge receipt of a copy of this Contract and an explanation of co-signer obligation form.                                                OWNER:

NOTICE OF RIGHT OF CANCELLATION

You may cancel this transaction, without any penalty or obligation, within three business days from the above date.  If you cancel, any property traded in, any payments made by you and any negotiable instrument executed by you under the contract or transaction will be returned within 10 business days following receipt by the Seller of your cancellation notice, and any security interest arising out of transaction will be canceled.

If you cancel you must make available to the Seller at your residence, in substantially as good condition as when received, any goods delivered to you under this contract or transaction, or you may, if you wish, comply with the  instructions of the Seller regarding the return shipment of the goods at the Seller’s expense and risks.

If you do make the goods available to the Seller and the Seller does not pick them up within 20 days of the date of your Notice of Cancellation, you may retain or dispose of the goods without any further obligation.  If you fail to make the goods available to the Seller, or if you agree to return the goods to the Seller and fail to do so, then you remain liable for performances of all obligations under the contract.
